Lemegeton is an unlockable activated item added in The Binding of Isaac: Repentance. It is the starting item for Tainted Bethany.
Effects
- Upon use, Lemegeton will grant a random passive item as an orbital familiar. The item summoned has a 25% chance to be from the current room's Item Pool, or from a random item pool otherwise. The item must have the '
summonable
' item tag in order to be generated.- These familiars can block up to 4 shots and/or get hit by 2 explosions before being destroyed. When the familiar is destroyed, the item it granted is lost.
- If Isaac is shielded or invulnerable, his familiars are also immune to damage.
- Familiars also deal contact damage to enemies equal to 2× Isaac's damage per tick (4× Isaac's damage per second). They take contact damage.
- Isaac can have up to 26 familiars: an inner counterclockwise ring of 6, a middle clockwise ring with 8, and an outer counterclockwise ring of 12.
- Using Lemegeton while Isaac already has 26 causes the oldest one to be replaced.
- These familiars can block up to 4 shots and/or get hit by 2 explosions before being destroyed. When the familiar is destroyed, the item it granted is lost.
- This item belongs to the Bookworm set. Collecting three items from this set will transform Isaac into Bookworm.
Notes
- Tainted Bethany starts with this item in the consumable slot.
- When playing as Tainted Bethany, picking up 4.5 Volt will cause Lemegeton to no longer gain charges as long as her normal active item has empty charges.
- Items given by Lemegeton are separate from the normal item pools and Isaac's current collection of items. This has the following effects:
- Isaac can still normally find items that were previously summoned by Lemegeton.
- Isaac can have multiple wisps of the same item.
- Wisps cannot be rerolled by Missing No. or the D4 / D100.
- Wisps do not count towards transformations.
- Super Bum is an exception and can be gotten from wisps.
- Summoned items do not count towards the Collection Page.
- Items that would normally add or remove hearts/consumables (such as Boom!, Breakfast, Abaddon, etc.) do not do so if given by Lemegeton.
- Summoning a Birthright wisp grants Tainted Lost infinite lives until the wisp is destroyed.
Random pool choices
When Lemegeton chooses an item pool different than the current room's, the chances are as follows. In Greed Mode, the mode's own pools are used except for the Planetarium. Probabilities are rounded to nearest 0.1%, so the totals may not be exactly 100%.

Synergies
- BFFS!: Doubles the damage of Wisps.
- Book of Virtues: Lets Lemegeton's wisps shoot homing tears once per second that deal 3 damage.
- Also applies to Tainted Bethany's Lemegeton, despite not being visually shown.
- Rock Bottom: Stat increases remain even after its wisp has been destroyed.
- Satanic Bible: Using Lemegeton in a Boss Room after activating the Satanic Bible on that floor will pull from the Devil Room item pool, even if the boss wouldn't normally drop an item after defeat.
Interactions
- Box of Friends: Does not double the wisps spawned by Lemegeton.
- Car Battery: Two wisps are created on use, only the name of the second item is displayed. If a Car Battery wisp is created, another wisp is summoned immediately.
- Chaos: Item wisps will be selected from random pools, regardless of the room Isaac is in.
- Magic Skin: Can be spawned by Lemegeton if it has been used in the current run.
- Sacred Orb: Generated wisps are affected by Sacred Orb.
- Sacrificial Altar: Every wisp will be converted to a Half Soul Heart. Familiars created by Lemegeton are sacrificed normally.

Trivia
- The Lemegeton is another name for the Lesser Key of Solomon, an anonymous grimoire focused on demonology that was compiled in the 17th century.
- The symbol on the cover of Lemegeton may be the Seal of Solomon.
Bugs
Bug! | After using Genesis, Isaac keeps the passive effects of each wisp until the next use of Lemegeton, which erases all lingering wisp effects. |

Bug! | If a run is exited and resumed while Isaac has item wisps generated from Lemegeton, all wisps will be condensed into the middle clockwise ring, even if this would result in more wisps on that ring than normally allowed. This also allows further wisps to be generated on the inner and outer rings. |

Bug! | Each use of Glowing Hourglass that does not move Isaac to a different room from the one he is currently in (such as by teleporting to the previous room then using it again) will duplicate the wisps. Creating wisps in this way allows Isaac to exceed the item's wisp limit. |
