Effects
- Prevents low-quality items from appearing, greatly increasing the quality of items received from all item pools.
- Items with a quality of 0-1 are automatically rerolled.
- Items with a quality of 2 have a 33% chance to be automatically rerolled.
- Quest items cannot be rerolled.
- This item belongs to the
Seraphim set. Collecting three items from this set will transform Isaac into an angel.
Notes
- Due to being quality 0,
Skatole cannot be obtained from 
Shell Games after Sacred Orb has been picked up.
Interactions
Bag of Crafting: Sacred Orb prevents crafting quality 1 and 0 items, typically resulting in them being replaced by quality 2 items. If a recipe that can only result in a quality 0-1 item is put in the bag, it will replace the item with Breakfast instead.
Binge Eater: The food item is not re-rolled.
Death Certificate: Death Certificate's item generation ignores Sacred Orb's changes to item availability.
Dice Bag: The one-time-use dice given by this trinket are not affected.
Genesis: If obtained from one of the Genesis room pedestals, no effect on future pedestals spawned in the room. Will function normally upon leaving.
Lemegeton: Generated items are affected by Sacred Orb.
Modeling Clay: Temporary items generated by this trinket are affected by Sacred Orb.
Sacred Orb: Additional copies of this item provide no benefit. The chance of quality 2 items being rerolled does not increase.
Spindown Dice: Spindown Dice ignores Sacred Orb's changes to item availability.

- The design of the Sacred Orb seems to be based upon a globus cruciger, lit. "cross-bearing orb", a Christian symbol of authority.
- This item is a stronger version of
Tainted Lost's "better items" mechanic. Its unlock method is unusual, as most other tainted characters have items based on their unique mechanics unlocked by defeating 
Delirium instead of 
The Beast.
