Proptosis is a passive item.

Effects

  • (except in Repentance and Repentance+) Doubles Isaac's damage stat.
  • (in Repentance and Repentance+) +0.5 damage.
  • Tear size multiplier: 2x.
  • (except in Repentance and Repentance+) The tears shrink and lose damage the farther they travel.
  • (in Repentance and Repentance+) The tears shrink and lose damage the more time they stay airborne, starting at 3x damage at point blank range. After 0.8 seconds, they deal no damage at all.

Notes

  • (except in Repentance and Repentance+) Effective range of Isaac's tears now also depends on the damage, as they can shrink out of existence before their air time runs out.
  • (in Repentance and Repentance+) Since tears run out of damage after 0.8 seconds (48 frames), shot speed is the only stat that determines the effective range of Isaac's tears. Damage and range don't affect the travel distance at all.
  • (in Repentance and Repentance+) Approximately 2.08% damage is lost per frame. This means at approximately 0.53 seconds (32 frames) Isaac's tears will deal the normal amount of damage.
  • (except in Repentance and Repentance+) Tear size increasing items also increase the maximum distance before its disappearance, but the damage dealt can go below original at the extreme.
  • (except in Repentance and Repentance+) The damage decrease does not affect Character Azazel icon.pngAzazel's short-ranged BrimstoneBrimstone unless the range stat is extremely high.

Synergies

  • Anti-GravityAnti-Gravity: The tears will not decrease in size or damage until they start moving, making this excellent against enemies that chase Isaac.
    • Anti-Gravity + (in Repentance and Repentance+) Eye of the OccultEye of the Occult: Lets Isaac maintain the full 3× damage multiplier while controlling his tears.
    • Anti-Gravity + Spoon BenderSpoon Bender: Tears will move towards enemies while maintaining the full 3× damage multiplier, negating both the need to release tears and the power decrease with distance.
  • BrimstoneBrimstone: Retains the double damage effect without losing damage over range.
    • (except in Rebirth) Damage is doubled again for a total of 4x next to Isaac, but quickly drops further along the beam, at about 0.8x per tile of distance.
    • (in Repentance and Repentance+) The beam does 6x damage at point blank range.
  • (in Repentance and Repentance+) C SectionC Section: Fetuses deal triple their usual damage (2.25× Isaac's damage) at point blank, but shrink and deal less damage over time.
    • C Section + Mom's KnifeMom's Knife: The knives shrink along with the fetuses, but they retain their tripled damage.
    • C Section + Tech XTech X: The rings shrink along with the fetuses, but they retain their tripled damage.
    • C Section + TechnologyTechnology: Although the laser's damage diminishes along with the fetus, it does not lose its damage over range.
  • (except in Repentance and Repentance+) Dr. FetusDr. Fetus/Epic FetusEpic Fetus: Doubles damage with no penalty for distance.
    • (in Repentance and Repentance+) Triples/quadruples damage respectively.
  • GodheadGodhead: The damaging halo shrinks with the tears.
  • A Lump of CoalA Lump of Coal: Proptosis scales with Isaac's damage while the Lump of Coal does not. At base damage, the Lump of Coal will approximately negate the damage lost over distance, but with increased damage, tears still do more damage up close.
    • A Lump of Coal + The Ludovico TechniqueThe Ludovico Technique: The controlled tear has full Proptosis damage buff, (except in Repentance and Repentance+) no matter how far it is from Isaac. (in Repentance and Repentance+) Very slightly decreases damage from far away.
  • Mom's KnifeMom's Knife: The Knife damage is (except in Repentance and Repentance+) doubled ((in Repentance and Repentance+) tripled), regardless of range.
  • PolyphemusPolyphemus: Tears do massive damage and carry with leftover damage while still shrinking.
  • TechnologyTechnology/Technology 2Technology 2: Doubles damage.
    • (except in Rebirth) The beam decreases in damage and width the farther it travels. Tears become virtually powerless across large rooms like the Boss Rush and the Hush fight.
    • (in Repentance and Repentance+) The beams do 6x damage at point blank range.
  • (except in Rebirth) Tech XTech X: Ring size and damage is substantially increased, but decreases as the ring travels further from Isaac.
  • (in Afterbirth †) TrisagionTrisagion: Base damage is doubled and beam damage is boosted to about 65%. Blasts lose no power as they travel further, though the tips of the blasts deal very slightly less damage.
    • (in Repentance and Repentance+) Damage slightly increased, beam damage is doubled. Blasts lose no power with distance.
  • (in Repentance and Repentance+) Bird's EyeBird's Eye: Size and damage of fires is increased, but does not decrease with range.

Interactions

  • (except in Rebirth) AthameAthame/(except in Rebirth) Maw Of The VoidMaw Of The Void: The ring is much larger and deals 1.5× damage.
    • (except in Rebirth and Afterbirth) The ring deals normal damage.
    • (in Repentance and Repentance+) The ring is now normal size.
  • (in Repentance and Repentance+) Broken WatchBroken Watch: The speedup effect applies to tears and increases the effective range.
  • (except in Rebirth and Afterbirth) Compound FractureCompound Fracture/Cricket's BodyCricket's Body/The ParasiteThe Parasite: Tears will often disappear before they can burst. Tears created from bursts will also shrink, but due to their already low range, the difference may not be noticeable.
  • (in Repentance and Repentance+) Era Walk card/I'm Drowsy... pill/Stop WatchStop Watch: The slowdown effect applies to tears and reduces the effective range of Proptosis.
  • (except in Rebirth and Afterbirth) Flat StoneFlat Stone: At base range and tear size, tears will bounce twice against the floor and then disappear.
  • (in Repentance and Repentance+) Immaculate HeartImmaculate Heart: Tears that appear around Isaac disappear quickly.
  • IpecacIpecac: Tears almost disappear completely but are still able to do massive damage.
    • (in Repentance and Repentance+) Tears no longer deal massive damage at the end of the arch. Damage decreases rapidly with range.
  • Isaac's HeartIsaac's Heart is a very good item to have, as you can run up to most enemies to deal maximum damage to them with little downside.
  • (except in Rebirth and Afterbirth) LachryphagyLachryphagy: Tears disappear too quickly to be fed.
  • The Ludovico TechniqueThe Ludovico Technique: The closer the tear floats to Isaac, the larger it is. The farther away, the smaller it becomes.
  • Monstro's LungMonstro's Lung/(except in Rebirth and Afterbirth) Lead PencilLead Pencil: Tears will disappear at different distances depending on how big they were when fired.
  • My ReflectionMy Reflection: Tears disappear before they can return to Isaac.
  • (except in Rebirth and Afterbirth) Pop!Pop!: Tears will disappear extremely quickly after being fired.
  • (in Repentance and Repentance+) SaturnusSaturnus The ring tears from Saturnus disappear nearly instantly upon entering a room.
  • Soy MilkSoy Milk: Very poor combination. Tears disappear nearly instantly in front of Isaac, decreasing his range and damage to nearly nothing.
    • (in Repentance and Repentance+) Tears no longer disappear instantly. Range is only minimally shorter.

  • Eye proptosis is a condition resulting in forward displacement and entrapment of the eye from behind by the eyelids. The condition is also known as eye dislocation and eye luxation.
