Familiars that follow behind Isaac have a certain priority in how close their position is to him over other familiars, with Demon Baby having 5th priority.

Box of Friends: If used without having any familiars, a Demon Baby will be spawned for the remainder of the current room.
Chocolate Milk: Shoots at enemies while charging up a shot, thus giving Isaac an alternative way to kill enemies while charging a shot.
Guppy: Spawns flies as if normal tears are hitting enemies.
Mongo Baby: Behaves like Demon Baby, and will no longer shoot its regular tears unless Isaac has another compatible familiar. If Isaac has other familiars that Mongo Baby can mimic, its behavior alternates between Demon Baby’s and the other familiars’.
