Little Baggy is a passive item.

Effects

  • Allows Isaac to carry two pills. Tapping the Drop button will cause a cycle through the two pills.
  • Spawns a random pill.
  • (except in Repentance and Repentance+) Changes all card and rune drops into pills.
  • (in Repentance and Repentance+) Changes all card drops into pills. Existing runes will not be converted, but new runes will only appear through forced drops (such as from (in Repentance and Repentance+) Mom's RingMom's Ring or (in Repentance and Repentance+) Clear RuneClear Rune).

Notes

  • Any Tarot card or rune held when picking this item up will change into a pill.
    • (in Afterbirth and Afterbirth †) Dice ShardDice Shard counts as a card and will be converted.
    • (in Repentance and Repentance+) If playing as (in Repentance and Repentance+) Character Jacob and Esau icon.pngJacob and Esau, if one character picks up Little Baggy while the other character holds a card, that card will remain until used or dropped.
  • (in Repentance and Repentance+) Blowing up the skull marked with a small "X" in DepthsDepths will drop Telepills instead of 0 - The Fool0 - The Fool.
    • If The Fool has already been generated, the generated pill will not be guaranteed to be a Telepills.
  • (in Repentance and Repentance+) Tainted Characters who have an active item in the consumable slot will be allowed a third slot for pills.

Interactions

  • Blank CardBlank Card: All cards are replaced by pills, rendering Blank Card useless unless Isaac also has Starter DeckStarter Deck.
  • Crystal BallCrystal Ball: Generates pills instead of cards upon use.
  • Deck of CardsDeck of Cards: The item becomes functionally identical to Mom's Bottle of PillsMom's Bottle of Pills.
    • (in Repentance and Repentance+) During multiplayer, if Little Baggy is held by one player, other players can still spawn cards using Deck of Cards. Dropping the card will convert it into a pill.
  • (in Repentance and Repentance+) False PHDFalse PHD: Since all cards turn to pills, there is potential for a lot of damage ups.
  • (in Repentance and Repentance+) Lucky FootLucky Foot: Most negative pills will be converted to their positive counterparts.
  • PHDPHD/VirgoVirgo: Most negative pills will be converted to their positive counterparts.
  • (except in Repentance and Repentance+) Starter DeckStarter Deck: Overrides Little Baggy. Pills may still be for sale in shops, but dropping them turns them into cards.
    • (in Repentance and Repentance+) Having both items allows both cards and pills to appear again.
      • Picking up Starter Deck while already holding Little Baggy will turn any held pills into cards.

Bugs

Bug Bug! (in Repentance and Repentance+) Patch V1.7.5 stated that "Little Baggy no longer affects pocket items that are neither pills nor cards (such as runes)", even though that is not the case.
