Ouroboros Worm is a trinket added in The Binding of Isaac: Afterbirth †.
Effects
- Tears travel in a large spiral pattern.
- Grants spectral tears.
+4.0 range, +2.0 tear falling speed.
+1.5 range, +0.4 tears.
- A luck-based chance for a homing shot, reaching 100% at 9 luck.
- The homing shots are not different in appearance.
- The homing does not override the spiral pattern, meaning tears stay unreliable.
Notes
While in the Mirrored World, the spiral pattern will also be mirrored.
- This also applies to the
Ring Worm trinket.
- This also applies to the
Synergies
Golden Trinket /
Mom's Box: The spiral pattern is larger, tears travel faster and the stats increases are doubled.
Aquarius: Creep has a chance to be homing.
Dr. Fetus: Since the bombs don't follow the spiral pattern, only the extra range, spectral bombs, and chance for a homing bomb apply.
Tractor Beam: The tears follow a straight path, ignoring the spiral pattern; the tears also stall at a specific point, giving a greater chance to land tears.
Interactions
Azazel /
Brimstone /
Technology /
Technology 2: The spiral effect is overidden. All other effects still apply.
The laser is shot in a spiral pattern. Homing does take effect, but is hardly noticeable.
The Ludovico Technique: The tear spirals around in a difficult to control manner, with the added homing chance further complicating things.
Mom's Knife: Overrides Ouroboros Worm.
Works as intended. The homing, however, is questionable.
Proptosis: The curious pattern allows dealing full damage from a safer range.
Tainted Azazel: His Brimstone laser is shot in a spiral pattern. Homing does take effect, but is hardly noticeable.
Tech X: Overrides Ouroboros Worm.
Tech X now inherits the homing chance benefit, without being affected by the pattern.

Trivia
- This trinket is a reference to Øuroboros, which was a prototype of the game "The End is Nigh" by Edmund McMillen and Tyler Glaiel.
- The Ouroboros is an ancient symbol, showing a serpent or dragon eating its tail.
