Brain Worm is a trinket added in The Binding of Isaac: Repentance.
Effects
- Tears snap 90 degrees to target enemies that they may have missed.
Notes
- The effect will not work if Isaac is too close to enemies.
- The effect will only work if the tear has a line of sight to the enemy: if it's interrupted by an obstacle, Brain Worm won't work even if Isaac has enough tear height to otherwise clear it unless he also possesses spectral tears.
Interactions
Anti-Gravity: Tears will snap to enemies after they're released, but not while the tears are staying in place.

Technology 2: Laser will snap regardless of length from enemy. Also works with 
Azazel's Brimstone.
Cricket's Body / 
The Parasite: Both the main and split tears are affected by Brain Worm.

Eye of the Occult: Tears briefly snap to target enemies but Isaac's control quickly takes over them.- Homing tears: Override Brain Worm.
The Ludovico Technique: No effect.
Mom's Knife: No effect.
Tech X: No effect.
Tractor Beam: No Effect.

- Brain Worm originates from the Antibirth mod.
- The name, appearance and effect of the trinket are likely inspired by the
Brain enemy. - The trinket might be a reference to Parelaphostrongylus tenuis, a real life brain worm commonly found in white-tailed deer.
