It doesn't work for me.

For me too...i wonder if it's bugged or has other effect...

Ok it works now, I tested it in my last game. It drops randomly a red heart (half or full), a coin, a bomb or a key (single or 1+1), every 3 rooms.

It seems to be dropping trinkets as well, at least when I had it in Greed Mode with the afterlife dlc...
