Getting multiple balls of bandages will evolve it just like ball of meat, I assume 3x means you get the girl from meat boy just like you get meat boy with 3 cubes of meat.
I only got 2, which turned the ball into a smiling head with a flower on it.

There is a problem with BoRev and map revelation though. A large boss room, that is discovered (probably only possible before the the usage of the Book) before a book usage will prevent a Horseman from spawning.
