Isaac's Tomb is an unlockable passive item added in The Binding of Isaac: Repentance.
Effects
- Spawns an
Old Chest at the start of each floor. - Old Chests are similar in appearance to
Locked Chests and likewise require a 
Key to unlock, but have the color of a normal Chest and a worn-down appearance. When opened, they contain one of the following:
- 1-3
Soul Hearts (43% chance) - 3 trinkets (42% chance)
- An Old Chest item (10% chance)
- An Angel Room item (5% chance)
- 1-3
Notes
- The chest also spawns in the mirrored floor of
Downpour II and 
Dross II if picked up before entering. - During the Ascent, the chest spawns in the original starting room of each floor, right above the beam of light to the floor above.
- The chest does not spawn in
Home. - Does nothing on
Tainted Lazarus if the new floor is entered by the character without Isaac's Tomb.
Synergies
Death Certificate : An Old Chest will appear in the Death Certificate area, unless Death Certificate is used on the floor where Isaac's Tomb was obtained.
Interactions
Crooked Penny / 
Diplopia: Additional copies of Isaac's Tomb have no effect. Only one chest is spawned at the start of the floor.
Gilded Key / 
The Left Hand / 
Mom's Key / 
Poker Chip: No effect.
