Fragmented Card only needs to be held when entering the floor. The Secret Room will remain after dropping the trinket, even if it hasn't been explored yet.
Golden Trinket / Mom's Box: One of the secret rooms is revealed on the map. If Isaac has both a golden trinket and the Box, both secret rooms are revealed.

XVIII - The Moon: The Moon will always teleport to an unexplored secret room. If one has already been explored, it will teleport to the other one. If both have been explored, it will teleport randomly in one of the two.
