This item belongs to the Beelzebub set. Collecting three items from this set will transform Isaac into a giant humanoid fly.
Notes
The Big Fan orbits independently rather than occupying one of the three defense orbital slots.
Pretty Fly Horse Pills spawn a Big Fan without granting it as an item. These Big Fans spin at normal pace and don't affect rotation speed of other orbitals.
Unlike regular Pretty Fly, there is no limit to how many familiars can be spawned this way.

BFFS!/Hive Mind: Big Fan becomes visually larger and deals more damage.
Guillotine: Reduces the head's rotation speed, making it easier to aim.
Guardian Angel: Overrides Guardian Angel's speed boost. If Guardian Angel was obtained first, it will retain its own increased speed. If Big Fan was obtained first, Guardian Angel will be slower.
Psy Fly: Rotates slower, but moves at full speed while blocking incoming projectiles.
