Backstabber is a passive item added in The Binding of Isaac: Afterbirth †.

Effects

  • When Isaac's attacks land on the "back" of enemies, they deal double damage and inflict bleeding.
    • (except in Repentance and Repentance+) Bleeding enemies take damage equal to 10% of their maximum health every 5 seconds. Bosses cannot be inflicted with bleeding, but will still take the extra damage.
    • (in Repentance and Repentance+) Enemies bleed for 5 seconds, bleeding enemies take damage based on how quickly they move; the faster the enemy, the more damage they take. While moving, a bleeding enemy leaves behind red creep that can damage other enemies. Bosses can also be inflicted with bleeding.
    • The extra damage is dealt even if the enemy is already bleeding.

Notes

  • What is considered to be the "back" of an enemy is based solely on their velocity when the attacks land: their back is opposite to their movement direction.
    • High knockback makes it easier to get follow-up backstabs as the enemies are pushed away from the tears.
    • Pushing enemies while shooting makes them easy to backstab, though is mostly viable only on ones that don't deal contact damage or while having a shield.
    • Completely stationary enemies generally have their back on their upper and right sides, but knockback considerations apply.

  • (except in Repentance and Repentance+) Shooting familiars: Tears fired by familiars can inflict the effects of Backstabber.

Trivia

  • This item is a reference to the backstabbing mechanic in various video games, where backstabbing results in more damage done to the target. However, the mechanic of backstabbing is usually applied to melee weapons, not ranged weapons.
    • It may also be a reference to Judas' betrayal of Jesus.
  • Backstabber was originally an unofficial item which was supposed to showcase the modding capabilities of Afterbirth+ in the PAX 2016 livestream. However, all items included in the demo were later added to the final game.
