???, also known as Blue Baby, is a character unlocked by defeating Boss Mom's Heart ingame.pngMom's Heart ten times. He starts with three Soul Heart.pngSoul Hearts and The PoopThe Poop.

??? cannot gain Red Heart containers; items that grant Red Heart containers (including empty ones) will give him the same amount of Soul Heart.pngSoul Hearts instead.

Notes

  • ??? has average base stats and has a smaller resource pool for health, as he cannot gain Red Hearts. This makes the character challenging as his Soul Hearts cannot be spent carelessly by taking damage or on sacrifices.
  • ???'s starting item, The PoopThe Poop, can provide powerful utility despite its weak appearance.
    • The Poop will knock back nearby enemies ((except in Rebirth) also knocks back pickups and enemy shots) upon use. Poops can also block enemies and low-flying tears.
    • Poops can bridge gaps when bombed from the opposite side (as if they were rocks).
    • Placing Poops on Red Poop.pngRed Poops when they are temporarily disabled (in Repentance and Repentance+) and retractable spikes when they are retracted will destroy them.
    • Poops spawned in this way will still yield pickups like normal.
    • (in Repentance and Repentance+) As ??? has an innate Lil LarvaLil Larva effect, it can be wise to use The Poop repeatedly in one room without destroying them at first, then clear rooms all the way to a Boss RoomBoss Room or other hard rooms, finally breaking all the Poop at once and taking on the hard room. This way, the Blue Flies will be put into better use, being effective if the player lacks damage.
      • A possibly unintended effect is that this stacks with multiple ???'s in true coop: along with The Poop, the potential flies gained per room increases quadratically with players, with the formula simply being the amount of ???'s in game to the second power.
    • The Poop will not work well with items or transformations that grant the ability to destroy obstacles on contact, such as Thunder ThighsThunder Thighs, LeoLeo, and (in Repentance and Repentance+) (except in Rebirth and Afterbirth) StompyStompyError: The given transformation ("stompy") does not exist. as the Poop spawned by the item will be destroyed instantly.
  • As ??? cannot have red health, Devil RoomDevil Rooms or Angel RoomAngel Rooms are guaranteed.
    • (in Repentance and Repentance+) Additionally, Devil Deals only cost their Red Heart equivalent in Soul Hearts, unlike other characters with no red health for whom the deals will always cost 3 Soul Hearts.
  • Sleeping on a bed will give ??? three Soul Heart.pngSoul Hearts.
  • (except in Rebirth and Afterbirth) Bone Heart.pngBone Hearts grant an empty Bone Heart upon pickup, but they cannot be refilled using Red Hearts.
  • The AnkhAnkh and Broken AnkhBroken Ankh will revive any character as ???, even before he is unlocked.

Item Interactions

Positive

  • (except in Repentance and Repentance+) AbaddonAbaddon costs three Soul Hearts and grants six Black Hearts, damage up, fear shot, and no negative effects to ???. Abaddon can be bought safely regardless of current health, because its price can be deducted from the acquired Black Hearts retroactively.
    • (in Repentance and Repentance+) AbaddonAbaddon will give 2 black hearts on pickup, essentially refunding the cost to obtain the item from a Devil Room.
  • D20D20 and (in Repentance and Repentance+) GB BugGB Bug can be used to reroll any red hearts that may drop into useful pickups.
  • AnkhAnkh is effectively a 1up!1up! for ???.
  • (in Afterbirth and Afterbirth †) AthameAthame can be used to produce black hearts upon taking damage.
  • Balls of Steel pills or V - The HierophantV - The Hierophant grant two soul hearts, both of which are more useful than normal HP up items.
  • DagazDagaz grants one soul heart as well as lifting any curses.
  • (in Rebirth) The XI - StrengthXI - Strength card gives ??? a permanent soul heart.
  • Book of RevelationsBook of Revelations grants ??? soul hearts.
  • Ceremonial RobesCeremonial Robes costs (except in Repentance and Repentance+) three (in Repentance and Repentance+) two Soul Hearts and grants three Black Hearts and damage up. Like AbaddonAbaddon, this item is always safe to take.
  • (except in Rebirth) Crown Of LightCrown Of Light will always activate when entering a room, but still deactivates after taking damage.
  • MitreMitre and Daemon's TailDaemon's Tail replace useless Red Hearts with Soul and Black Hearts respectively.
  • Dark BumDark Bum converts useless Red Hearts into Black Hearts and blue spiders.
  • Dead CatDead Cat will set Soul Hearts to 1, but can be used to gain devil deals by sacrificing the extra lives.
  • (except in Rebirth and Afterbirth) Dim BulbDim Bulb is able to be activated every room due to the single charge on The PoopThe Poop, providing a consistent stat boost.
  • (except in Rebirth) Empty VesselEmpty Vessel will always be active. (in Repentance and Repentance+) Additionally, it costs 1 Soul Heart and returns 2 Black Hearts, so the item is always safe to take.
  • GimpyGimpy has a chance to spawn Soul Hearts when hit.
  • (except in Rebirth and Afterbirth) Hallowed GroundHallowed Ground turns all The PoopThe Poop uses into holy poops.
  • Lazarus' RagsLazarus' Rags causes ??? to be resurrected as Character Lazarus Risen icon.pngLazarus Risen when he dies, allowing the gathering of heart containers. Once this happens, however, you are no longer playing as ???, and therefore cannot get any of his achievements/unlocks. The same effect happens with the use of Judas' ShadowJudas' Shadow.
  • (in Afterbirth and Afterbirth †) Maw Of The VoidMaw Of The Void can produce Black Hearts.
  • Midas' TouchMidas' Touch makes The PoopThe Poop create Golden Poop.pngGolden Poop.
  • Monkey PawMonkey Paw will drop 3 Black Heart.pngBlack Hearts when at half a heart, but must take damage for each one to spawn.
  • Petrified PoopPetrified Poop is useful early on to get more pickups from The PoopThe Poop every room.
  • (in Repentance and Repentance+) The MarkThe Mark can be bought safely in all circumstances since it costs 1 Soul Heart and returns 1 Black Heart.
  • The PactThe Pact is another item that is always safe to take, as it costs (except in Repentance and Repentance+) three (in Repentance and Repentance+) two Soul Hearts and grants two (except in Repentance and Repentance+) Soul/(in Repentance and Repentance+) Black Hearts on pickup, alongside a damage and tears up.
  • The PolaroidThe Polaroid and The NegativeThe Negative will always activate when taking damage.
  • The RelicThe Relic, which is generally useful, is especially useful to ??? as it spawns Soul Hearts.
  • Satanic BibleSatanic Bible generates Black Hearts.
    • (in Repentance and Repentance+) Using Satanic Bible now also causes boss items to turn into devil deals that cost hearts, which is often worth since Devil deal items are cheap and better than boss items.
  • (except in Rebirth) Serpent's KissSerpent's Kiss can produce Black Hearts by poisoning enemies.
    • (in Repentance and Repentance+) Serpent's Kiss now only produces Black Hearts from contact damage poison, making it slightly less useful for ???.
  • SqueezySqueezy and (except in Rebirth) Mr. DollyMr. Dolly increase ???'s tears and can grant Soul Hearts.
  • Whore of BabylonWhore of Babylon is always active.
  • (in Repentance and Repentance+) BirthrightBirthright will double the effectiveness of HP upgrades, meaning ??? gains twice as many soul hearts as normal.
    • This can be exploited with ConverterConverter to generate unlimited HP.
  • (in Repentance and Repentance+) Fortune CookieFortune Cookie can give a soul heart upon use.
  • (in Repentance and Repentance+) IBSIBS spawns different kinds of poop, all of which spawn blue flies when ??? destroys them.
  • (in Repentance and Repentance+) Judas' TongueJudas' Tongue lowers the price of Soul Heart Devil deals.

Negative

  • Items that directly add Red Hearts to the health bar, such as Yum HeartYum Heart, (in Repentance and Repentance+) Yuck HeartYuck Heart or full health pills, are useless. However Eternal Heart.pngEternal Hearts still act as an extra half a heart and become a full soul heart at the end of the floor making Prayer CardPrayer Card viable.
  • Blood Donation Machine.pngBlood Donation Machines and Devil Beggar.pngDevil Beggars are riskier, as they deplete health that is harder to replenish.
  • (except in Repentance and Repentance+) Cursed SkullCursed Skull will automatically teleport ??? out of the room upon taking any damage, as he doesn't have any Red Hearts. This can be used to easily warp out of the Boss RushBoss Rush room, but is mostly an annoyance.
    • (in Repentance and Repentance+) Works only at half a soul heart.
  • (in Afterbirth and Afterbirth †) Glass CannonGlass Cannon will bring ??? down to half a Soul Heart, and not give back any health upon leaving a room due to ???'s lack of Red Hearts. This item essentially takes away most of the player's health in exchange for a single high damage tear.
    • (in Repentance and Repentance+) This effect no longer applies in Repentance, making it a more viable choice. Even so, caution should be used, as taking damage will cause the next damage taken in the room to be two full soul hearts, making this especially risky for ???.
  • Guppy's PawGuppy's Paw, since requiring Red Hearts to convert, would be a useless item to hold (but should still be picked up and swapped back to get credit towards the GuppyGuppy transformation).
  • Little C.H.A.D.Little C.H.A.D. is useless unless paired with Dark BumDark Bum or (in Repentance and Repentance+) Apple of SodomApple of Sodom, where it becomes very useful.

  • ??? appeared in the original Binding of Isaac, with the same unlock method.
  • The name "Blue Baby" was used in the original Binding of Isaac, where it would appear in Isaac's Last Will in parentheses after dying as him. This was removed in Rebirth.
    • ??? is referred to as "Blue Baby" in the game files and in The Binding of Isaac: Four Souls. It is also used for the name of a co-op baby that resembles him.
    • The name is a reference to blue baby syndrome, in which a baby's skin complexion appears blue from a lack of oxygen in its blood due to a congenital heart defect.
  • ???'s design originally came from Dead Baby Dressup!, one of Edmund McMillen's first Flash games.
    • Many of Edmund's older flash games on Newgrounds feature ??? and Brother BobbyBrother Bobby. ??? was Edmund's mascot from 1996 to 2002.
    • Edmund's username on Newgrounds is 'Bluebaby'.
  • In several cutscenes, Isaac is shown suffocating inside a chest. ??? is first seen inside an identical chest, and Boss ??? ingame.png??? is the follow-up boss after Boss Isaac ingame.pngIsaac. Blue skin tone is typically used to show suffocation, and cross-shaped eyes imply that the character is deceased.
    • After death, the bowels of the deceased will loosen, causing them to defecate. This is most likely the reason ??? starts with The PoopThe Poop.
  • ??? does not appear in the Bible or any religious scripture. He is the first character with this distinction, and the only one to appear in the original Binding of Isaac — the second being Character The Lost icon.pngThe Lost in Rebirth, third being (except in Rebirth) Character Keeper icon.pngKeeper in Afterbirth, and fourth being (except in Rebirth and Afterbirth) Character The Forgotten icon.pngThe Forgotten in Afterbirth+.
  • ???'s starting health is listed as "-I" on the character selection screen.
  • As a secret character, ??? does not appear on the character select screen until unlocked.
